The following is a list of forms that are required in order to register for
LB Crew. You can download a copy of any of the forms by clicking on the form
name.
LBSS Crew
Participation Registration & Payment Program
- Explains the
registration and payment schedule for both the Crew Pre-Conditioning
Program and the Crew Participation Program.
LBSS Crew
Registration Form
- Requests the
necessary information to register for the Crew Program. Must be completed
and submitted with the appropriate registration fee (see Crew
Participation Registration & Payment Program) by the November registration
meeting. See the calendar for the exact date.
Athletic
Participation/Parental Consent/Physical Exam Form
- Required by Fairfax
County School System, this form must be completed fully and signed by the
Doctor’s office and the parent by the registration dates if any student is
to participate in any athletic program. Download the latest Athletic
Participation Form via the link above or a blank form is available in the
LBSS main office.
If the student has participated in another 2009 LBSS sport where this form
has been submitted, then he/she does NOT need to provide another copy.
Emergency Care Form
- Required by Fairfax
County School System, this form must be completed fully by all rowers by the
registration dates. Copies of these forms are maintained in the erg room
for winter conditioning; and at Bull Run Marina once on-the water training
starts. Copies are also taken to all regattas.
Chances are you have already completed this form several times - the
school has a copy, and one has to be provided for all activities (e.g.,
band). Still, you need to provide yet another copy for crew. As noted
above, the care forms go everywhere with us.
LBSS Student
Participation Policy Form
- Required by Fairfax
County School System, this form must be read and signed by both the
student and, if the student is under 18, by a parent, if the student is to
participate in any athletic program. A copy for each program joined has to
be signed.
Please note that only the page with the signatures needs to be returned
with the registration material. A blank form is available in the LBSS Main
Office.
LBSS Crew Team
Guidelines
- The LB Crew Team
Guidelines spells out the expectations of coaches and athletes. Each
student is expected to be familiar with the guidelines and must sign a
form acknowledging that they have read the guidelines..
U.S. Rowing Insurance
Waiver Form
- This waiver form
indicates that the student and his/her parents understand rowing
activities and believe the student is qualified to participate in this
program. It indemnifies, saves, and holds harmless each of the Releasees
from any claim by the participant. No one may join any Crew practice or
regatta without a completed form in the Coach’s file.
