Lake Braddock Crew is open for registration from our interest meeting,
held in late October to the second General Membership meeting held in
mid-January. Check the calendar for the exact dates.
In order to register, students must fill out the following forms:

All of these forms have been
combined into one single PDF file called the LBSS Crew Registration
Package. This can be downloaded by
clicking here.
If you are having problems
printing the registration package - in particular, if some pages
contain only a part of the form, but are super-magnified - then be
sure on the Print dialog box that you set "Page Scaling" to "Fit to
Print Area".
Instructions for Registering:

Download the LBSS Crew Registration Package by
clicking here. Print out everything in the document and complete
all forms. Please note the following:
* Although you have already provided the school with a completed FCPS
Emergency Care form, you MUST provide yet another copy in order to
register for Crew.
* Note that Part III of the VHSL
Athletic
Participation/Physical Exam Form requires that each student have a
physical exam, and it must be signed by a physician. If you already
have a VHSL Physical exam on file with the school for a Fall or Winter
sport, you do NOT have to provide another copy. Include in your
registration packet a note indicating that you already have a VHSL
form on file with the school, and what sport if was submitted for.
However, if you DO NOT have a VHSL form on file NOW is the time to
schedule the appointment with your family doctor for a physical!

Bring all registration forms to the LBSS Crew General
Membership and Registration Meeting, currently scheduled for Thursday,
November 12, 2009. (Check the Calendar for any updates.) All
materials are to be turned into
the Registration Coordinator.

Alternatively, you can place all of
your forms (and your check, payable to Lake Braddock Crew Boosters) in
an envelope and place it in the Crew Box in the Student Activities
office. Please clearly label the outside of the envelope
"Registration". This will help keep it separate from "Spiritwear",
"Uniforms" and "Letter Writing Packets".
